Here is what I usually use on every fishing trip I go on:
5 foot ultra lite rod
Spinning reel- Rhino brand is my Favorite
4-6 pound line- Trileene or spider wire is my favorite
1/16-1/8 ounce rooster tail spinners (I have too many colors to list)
1/16 ounce rapala jerk bait- I preffer red or blue
size 8 hooks with a small 1/32-1/16 ounce weight or a bobber for live bait.
Waders
Fishing Vest- White Water are my favorite vests
Hat/Glasses- I like the Bass Pro brands the most
Small fish net- Bass pro brands, I like the kinds you can attach to your vest or waders so you don't have to carry it all the time.
Mister Twister Plastic worms- black, orange, green, white, brown, yellow, tan, and gray.
well that's it, I usually fish from the shore, but in rivers, I use my waders, but I use my vest all the time when I fish, I just find it much easier than having to carry a big tackle box with me all the time, when I can just unzip a pocket and get out a lure.

Worms! Plastic worms. Oil colored with blue metal-flake. Caroline rigged with 1/8 oz slip weight. #3 hook. 17 lb test line on bait casting reel. Short medium rod to keep it out of the bushes and trees. This is what I fish most of the time.

wow, you like to use heavy tackle, I always use light tackle, unless if I'm ocean fishing, or cat fishing, I'll use a heavy ugly stick rod, with a large spinning reel with 80 pound line, 2 ounce weight, and octopus hooks for ocean fishing or a trouble hook for catfish.
I try to make sure I never loose fish when I'm trying to get big ones.

I prefer to use what ever catches the fish! I recently started using medium action rods with baitcasting reels. I am learning techniques for this tackle and lures and as of yet haven't had much luck. I purchased a new light action Berkley Cherry Wood rod and Abu Garcia 102c reel and have caught my biggest fish this year with this set up. Funny, I started using medium action rods because I was not able to set the hook good enough with the light stuff I fished with for years. My biggest bass this year I would have lost if I was using heavy tackle as it was just barely hooked! You just never know! Bob

I just got back from pomme last week, and I have a huge tip for everyone, if you have a fly rod, use it!
Nothing was working as well as a fly rod did, I was fishing in the coves with a black bodied, white legged spider fly, it was about 1 inch long and about 1/3 inch wide, I was working it kinda fist through brush piles and big structures, but I worked it slow in deep, open water.
All the fish I caught last weekend were on a fly rod, all I caught with worms was a turtle.
